The effect of chronic paced single chamber right ventricular on atrio-ventricular valve function.

Abstract :

Aims and objectives : To assess effect of single chamber right ventricular pacing on function of atrio-ventricular valve at 1 year of prospective of follow up. Material and methods: This prospective study was undertaken to evaluate clinical profile, LV function, QRS morphology, and QRS duration of patients having atrio-ventricular disease, undergoing single chamber pacemaker implantation between November 2017 to January 2019 at UNMICRC, Ahmedabad, Gujarat. Results: In our study out of 135 patients, 98 patients completed 12 month follow up and were included in final analysis. Mean age was 65.92 9.64 years with 60.2% male. Hypertension (49.5%) was most common comorbid condtion . 35.7% and 40.8% patients had pre-syncope and syncope. About 16.3% and 7.1% patients had NYHA 2 and NYHA 3 grade dyspnea. At baseline, total 54 patients (55.1%) had MR. Out of them, 32 (32.7%) had mild MR, 18 (18.4%) had moderate MR, 4 (4.1%) had severe MR. During 6 and 12 month follow up proportion of moderate MR was similar but proportion of severe MR was significantly reduced to 0.0% (p=0.03). At baseline, total 65 patients (66.3%) had TR. Out of them, 35 (35.7%) had mild TR, 25 (25.5%) had moderate TR, 5 (5.1%) had severe TR. During 6 and 12 month follow up proportion of moderate and severe TR was not changed but proportion of mild TR was increased 35.7% from baseline to 53.1% at 12 month but statistically not significant (p=0.84). conclusions: No patients had developed congestive heart failure or atrial fiillation at 12 month of follow up. proportion of severe MR was significantly reduced. proportion of mild TR was increased but statistically not significant (p=0.84).
